Registration of deaths

  • Registration of death of Sri Lankan nationals occurred in Brazil or other countries in South America can be made at the Embassy of Sri Lanka in Brasilia, Brazil. The declaration for the registration of the death should be submitted within 3 months of such death by a nearest relative of the deceased.
  • The documents that are to be submitted by the declarant for the registration of death:
    • Duly perfected death registration form
    • If the application for registration of death is made within 3 months of the particular death, form “Registration B11 – Declaration of Death (under Section 30)“ should be used
    • If the registration is made after 03 months of death, form “Registration B15 – Declaration of particulars relating to death for purposes of late registrations (Section 36) should be furnished along with a letter of explanation with the reason for the late registration
    • Original Death Certificate of the deceased issued by relevant local authority in the country or the jurisdiction within which the death occurred.
    • The local death certificate along with its English translation must be duly attested by the Ministry of Foreign Affairs of the country/jurisdiction within which the death occurred or its diplomatic representation (Embassy/Consulate)
    • Doctor´s Medical Certificate/Coroner´s Certificate/Post-mortem/Autopsy Report where applicable
    • Original Birth Certificate of the deceased
    • Original Sri Lankan passport of the deceased
    • If the human remains have been buried/cremated in the country of deaths or elsewhere, a Burial or cremation certificate or any other authenticated report issued by the relevant authority in the country of which such burial or cremation performed
    • If the human remains have been repatriated to Sri Lanka for burial/cremation, the Airway Bill (Air Cargo Bill)
    • Original National Identification Card or the valid current passport of the declarant/informant of the death
    • Declarant of the death should be the next-of-kin (NOK) of the deceased person i.e. mother, father, spouse, child, or siblings of the deceased
    • Original Birth Certificate or Marriage Certificate of the declarant in order to establish the relationship between the deceased and the declarant
      • If the deceased is the spouse, the original Marriage Certificate of the declarant
      • If the deceased is the father/mother, the original Birth Certificate of the declarant
      • If the deceased is the child, the original Birth Certificate of the child along with the identification document i.e. passport of the declarant
    • Prescribed fees for the registration of deaths
  • The Certificate of Death will be issued only to the declarant, which can be collected in person either from the Embassy of Sri Lanka in Brasília, Brazil or the Consular Affairs Division of the Foreign Ministry in Colombo, Sri Lanka.
