Lost, Damaged or Stolen Passport

  • Sri Lankan nationals living in Brazil or other countries in South American continent whose passport reported lost or stolen can apply a new Sri Lankan ordinary passport through the Embassy of Sri Lanka in Brasilia, Brazil from the Department of Immigration and Emigration of Sri Lanka
  • Requirements for a new ordinary passport
    • Passport application Form K 35 A
    • Lost or Stolen Passport Complaint Form in case of lost or stolen
    • A Police Report obtained from the Local Police Station with its English Translation
    • A written Statement from the applicant regarding the loss of passport
    • Original passport or a clear copy of the passport/ passport number in case of lost or stolen
    • Original birth certificate
    • National identification card (NIC) or driving licence (DL)
    • 3 photographs
    • Valid visa card or permanent resident card
    • Citizenship confirmation/certificate
    • Prescribed passport fee

Instructions for new passport applications

  • In the event of a valid passport which is lost or stolen, a new passport can be applied.
  • Generally, it takes approximately 3 months to receive a new passport from Colombo after submitting an application to the Embassy along with other supportive documents.
  • The application for a new passport can be submitted to the Embassy by hand or post along with originals and photocopies of the required supportive documents.
  • If the application for the new passport is submitted by post the verification photograph affixed to the passport application and applicant’s signature should be attested by an authorised officer.
  • The verification photograph and the signature of the applicant can be attested by the Honorary Consul of Sri Lanka in the city/country of residence of the applicant, or by an authorised Public Notary in the area of the applicant’s residence.
  • All 3 photographs must be in colour with light blue or white background. The size of the photographs must be 3.5cm x 4.5cm which must be duly affixed to the passport application of the applicant. All 3 photographs should be taken within the last 3 months from the date on which passport application is submitted to reflect the current appearance of the applicant. Further, the photographs should be in full-face-view of the applicant which is taken directly facing the camera with natural facial expression. The forehead and both ears of the applicant must be clearly visible.
  • If the visa is not stamped on the passport of the applicant, the original of valid visa card or the permanent residence card issued to the applicant by the competent authority of the country of residence; or any other document which determines the visa status or the visa category of the applicant in the country of residence, must be submitted.
  • If the visa card or the permanent residence card is not available or expired; a non-acquisition of the citizenship by the applicant in the country of residence must be submitted. Such documents must be issued by the pertinent authorities in the host country.
  • If the applicant of the new passport was born outside Sri Lanka, and is a citizen of Sri Lanka by registration the original citizenship certificate issued by the Department of Immigration and Emigration of Sri Lanka must be submitted.
  • If the Citizenship Certificate is yet to be issued by the Department of Immigration and Emigration of Sri Lanka, the payment receipt issued in respect of the citizenship registration/Consular birth registration/certificate must be submitted along with the passport application.
  • If the applicant of the new passport is a married female who has changed her name after marriage, the original marriage certificate of the applicant must be submitted along with the passport application. (If the original Marriage Certificate is not in English, an English translation should be attached)
  • The marriage certificate must be duly authenticated by the Consular Affairs Division of the Foreign Ministry of Sri Lanka.
  • If the marriage was registered outside Sri Lanka and the marriage certificate has been issued by a foreign government, such marriage certificate must be authenticated by the Consular Affairs Division of the Ministry of Foreign Affairs of the country in which the marriage was registered/solemnized, or its diplomatic representation, i.e. Embassy or Consulate established in the applicant’s country of residence. (If the original Marriage Certificate is not in English, an English translation should be attached)
  • If the profession of the applicant is required to be included in the new passport, the original academic, professional or educational certificates or any other acceptable documents, such as a letter of appointment, recommendation issued by the employer or a professional institution with respect of the applicant´s profession must be submitted along with the passport application. (If the original document is not in English, an English translation should be attached)
  • The prescribed fee for the new Sri Lankan passport can be paid in cash at the Embassy or it can be transferred to the official bank accounts of the Embassy.
  • If the passport fee is paid through a bank transfer, a copy of the paid-deposit-slip must be submitted to the Embassy along with the new passport application.
  • Originals of all personal certificates/documents submitted to the Embassy will be returned to the applicant, after perusal/verification.
  • All applications for new passports along with relevant supportive documents are transmitted to the Department of Immigration and Emigration of Sri Lanka through the Foreign Ministry of Sri Lanka by the diplomatic mail bag of the Embassy of Sri Lanka in Brasilia, Brazil. However, due to the COVID-19 pandemic the applications for new passports are processed online.
  • Once the new passport is issued by the Department of Immigration and Emigration of Sri Lanka and received by the Embassy, the applicant will duly be notified of the same and he or she would be requested to collect the new passport from the Embassy.
  • If the return of the new passport is desired by mail, the applicant may choose to arrange a suitable courier service with a written notification authorizing the Embassy to surrender the new passport to the agent of the selected courier service along with applicant´s personal documents which were submitted to the Embassy along with the passport application
