Alteration of Passports/Amendments

Passport Amendments

Sri Lankan nationals reside in Brazil and other countries in South America can submit applications with the relevant documents to the Embassy of Sri Lanka in Brasilia in order to process the subsequent alterations/amendments in the passport and also inclusion or deletion of endorsements appeared in the respective the passport of the applicant.

1. Change of Name after marriage

  • Female applicants of Sri Lankan nationality who wish to have the spouse’s surname in her passport after marriage must submit the following document.
    • Written request by the applicant
    • Duly filled application form
    • Original passport of the applicant/female party
    • Original passport of the spouse/husband
    • Original Birth Certificate of the applicant
    • Original Marriage Certificate with English translation
    • Valid visa card or permanent resident card
    • Prescribed fee

Instructions

Original or copy of the marriage certificate must be authenticated by the Ministry of Foreign Affairs of the relevant country of origin or its diplomatic representation, i.e. Embassy, Consulate etc. if the marriage certificate is issued in Sri Lanka it should be authenticated by the Consular Affairs Division of the Foreign Ministry of Sri Lanka in Colombo.

2. Inclusion or Change of profession in the passport

  • Sri Lankan citizens reside in Brazil and other countries in South America who intend to include or change his or her profession in the passport must submit a written request to the Embassy of Sri Lanka in Brasilia along with the following documents.
    • Written request by the applicant
    • Duly completed application Form
    • Original Sri Lankan passport
    • Original professional certificate or documentary evidence to prove the profession of the applicant
    • Valid visa card or permanent resident card

Prescribed fee

Instructions

Original or copy of the professional certificate or employment records or similar proof must be authenticated by the Ministry of Foreign Affairs of relevant country of origin or its diplomatic representation. If such certificates/documents have been issued in Sri Lanka they must be authenticated by the Consular Affairs Division of the Foreign Ministry of Sri Lanka in Colombo.
